The competition exists out of two rounds, with each team conducting a lap of 400 metres in each round. Each athlete in a team takes part in a quarter of the race, with the fastest three teams in the heats advancing to the final, as well as the fastest non-qualifiers. The team with the fastest time in the final is the winner.

Sixteen teams qualified for the event with Jamaica and the United States traditionally as the strongest teams. The team of Great Britain was also one to look out for after winning silver at the world championships in 2017 and 2019, but the team of Germany had set the world leading time, just ahead of Switzerland. The team of the Netherlands was one of the rising teams, after winning the European championships in 2016, but the team of China could also surprise after having the third-fastest time of the year. Other potential medalists were Nigeria, Australia, and Canada.
