The competition exists out of two rounds, with each team conducting four laps of 400 metres in each round. Each athlete in a team takes part in a quarter of the race, with the fastest three teams in the heats advancing to the final, as well as the fastest non-qualifiers. The team with the fastest time in the final is the winner.

Fifteen teams qualified for the inaugural edition of the event with the United States as one of the big favorites, though with a new event, many other teams had medal chances. Jamaica won a silver medal at the most recent world championships, with Great Britain just missing out on a medal. A few months before the Olympic Games at the 2021 World Relays, the Dominican Republic took the bronze medal, just ahead of Belgium. Germany had the world leading time of 2021, with Ukraine and Nigeria within half a second, but other potential medal contenders and outsiders also included Poland, India, and the Netherlands.
