The competition exists out of two rounds, with each team conducting a lap of 400 metres in each round. Each athlete in a team takes part in a quarter of the race, with the fastest three teams in the heats advancing to the final, as well as the fastest non-qualifiers. The team with the fastest time in the final is the winner.

Sixteen teams qualified for the event with likely Jamaica and the United States as the big favorites, though Great Britain and Japan won the silver and bronze medals at the most recent world championships. China, Canada, and even Germany were also teams to be reckoned with fast times throughout the year, though only first former was able to reach the final at the 2019 world championships. Other potential outsiders also included Brazil, Italy, South Africa, and the Netherlands.
